2014—2015学年第一学期全区普通高中学业水平考试定于2015年1月10、11日举行，为切实做好考试准备工作,确保考试顺利实施，现将有关事宜通知如下：

一、考试时间【内蒙古招生考试信息网学业水平】

1月10日上午8:00—9:30 数学，上午10:30—12:00 外语（英语、日语、俄语），下午 15:00—17:00语文（蒙古语文甲、朝鲜语文）。

1月11日上午8:00—9:30生物，上午10:30—12:00历史，下午 15:00—16:30 地理。

二、考试对象及科目

2012级具有考籍的普通高中在校生开考语文、数学、外语三个科目。

2013级具有考籍的普通高中在校生开考生物、历史、地理三个科目；2012级参加普通高中学业水平考试生物、历史、地理科目成绩不合格者，可重新报考相应科目。

三、考籍注册

2014级普通高中学生须同时满足如下条件方可进行考籍注册：

1、参加过2014年全区中考统一网上报名且被区内所属普通高中正式录取的在校生。

2、符合高中毕业当年自治区普通高考的报名条件。

四、考籍补注册及审核

分别符合2015年、2016年自治区高考报名条件的2012级和2013级未注册考籍的普通高中在校生，可在规定时间内登录内蒙古招生考试信息网（

对区外务工人员随迁子女和户口从区外迁入我区的考生，各旗县区教育招生考试机构及各普通高中学校要严格按照内教办字〔2013〕25号文件规定对考生考籍注册资格、信息进行检查和网上审核（https://

注册时间、补注册时间和报考时间均为：2014年10月14日—31日，逾期不再办理。

已取得考籍并参加过该项考试的2012级考生，只须刷二代居民身份证和采集指静脉信息；已取得考籍未参加过该项考试的2013级考生，须使用全区统一前景及背景照相、刷二代居民身份证和采集指静脉信息。完成以上工作的考生方可登录内蒙古招生考试信息网（

2014级考生先在网上填写中考录取学校、就读学校、班级等录取信息完成注册手续，旗县区教育招生考试中心在完成2012级、2013级考生的信息采集工作后，再为2014级考生使用全区统一前景及背景照相、刷二代居民身份证和采集指静脉信息。2014级考生信息采集工作在11月14日18:00结束。

第一章 总 则

第一条 为保证我区普通高中学生学业水平考试工作的顺利实施，根据自治区教育厅《关于实行全区普通高中学业水平考试的通知》（内招考字[2011]4号）和《内蒙古自治区普通高中学业水平考试方案（试行）》(内教基字[2012]1号)文件的精神，制定本实施办法。

第二条 普通高中学业水平考试（以下简称“学业水平考试”）是在教育部指导下由省级教育行政部门组织实施的国家考试，旨在监测普通高中教育教学质量,全面反映学生各学科所达到的学业水平。

第三条 全区学业水平考试工作由自治区教育厅统一领导。自治区教育招生考试中心负责全区学业水平考试的报名、命题、制卷、考试、评卷、分析评价等各项组织管理和信息处理工作；盟（市）教育招生考试机构具体负责组织所属区域内的全区统一考试的考务工作以及本盟（市）统一考试(考查)的全面组织、实施和学校考试的管理、指导工作。

第四条 本《实施办法》适用于学业水平考试自治区统一考试部分。盟（市）（或学校）组织的考试（考查）部分的具体办法，由盟（市）根据本《办法》精神制定，并报自治区教育招生考试中心备案。

第二章 考籍注册和考试报名

第五条 高中新生入学后，新生所在学校组织学生在内蒙古招生考试信息网录入学生基础信息，注册建立学生考籍档案。

第六条 凡具有自治区学业水平考试考籍的学生都应参加学业水平考试，未取得考籍的学生不得参加学业水平考试。

第七条 学生获得学业水平考试考籍的同时即可获得学业水平考试准考证号。学生准考证号一经确定，一直使用到高中毕业。

第八条 考籍的建立、管理、变更和转籍要按照《内蒙古自治区普通高中学业水平考试考籍管理规定》执行。凡符合有关转学、休学、复学、退学等规定的学生，可以按规定进行相应的考籍变更。

第九条 学业水平考试报名在内蒙古招生考试信息网进行。盟（市）、旗（县、区）和学校按照提前公布的报名办法和设定的工作流程，在规定的时间内组织、指导学生网上完成填报考试科目、缴费等报名工作。报名工作结束后原则上不再安排补报名。

第十条 自治区教育招生考试中心确认所有信息后，盟（市）教育招生考试机构根据要求设定考点、编排考场，旗（县、区）负责准考证的打印、签章，并将准考证发放到各高中学校，考前高中学校发给学生。

准考证是考生参加学业水平考试的凭证，如有遗失或损坏，须由考生本人写出申请，经学校同意后由盟（市）、旗（县、区）教育招生考试机构审核后补发。

第三章 命题、制卷【内蒙古招生考试信息网学业水平】

第十一条 学业水平考试全区统一考试的科目为语文（蒙古语文甲）、数学、外语、物理、化学、生物、思想政治、历史、地理等9科的必修内容。盟（市）统一考试科目为信息技术、通用技术（待条件成熟后逐步纳入全区统考）,考查科目为物理、化学、生物实验和通用技术实践操作。学校考试科目为体育与健康、艺术（美术、音乐）、研究性学习活动、社区服务、社会实践活动和全区统一考试科目的选修部分。

第十二条 全区统一考试答卷采取闭卷、笔答方式。全区统一考试科目的考试时量:语文（蒙古语文甲）120分钟，数学、外语、物理、化学、生物、思想政治、历史、地理各90分钟；各科卷面分值满分均为100分。盟（市）、学校统一考试（考查）的内容、试卷模式、答卷方式、考试时间、考试时量及卷面分值由盟市、学校确定,原则上要依据课程方案和教学计划。

第十三条 学业水平考试命题工作由自治区教育招生考试中心组织具有丰富教学和命题经验的教师组成命题专家组具体负责实施。命题人员应具备的基本条件：（1）思想品德好，工作责任心强，组织纪律观念强，能严守国家机密。（2）业务素质好，专业水平高，有较丰富的命题和教学经验。（3）本人没有直系亲属在普通高中相应年级就读。（4）认真负责，身体健康。

第十四条 学业水平考试命题工作要统一标准、科学准确。以普通高中课程改革方案和课程标准为依据，适合模块教学特点，促进学生全面、健康发展。要紧密联系社会实际与学生生活，注重考查学生利用所学知识分析和解决问题的能力。

第十五条 自治区教育招生考试中心与每个命（审）题人员签订保密工作协议。命题人员应严格遵守保密协议和保密纪律，凡违反保密合约造成泄密的，一经查实，将按照国家有关保密法律、法规严肃处理。

第十六条 命题在国家规定的保密场所进行，入闱人员全封闭，直到考试结束。命题人

员必须严格遵守《命题工作人员守则》。

第十七条 学业水平考试全区统一考试科目试卷（试题卷和答题卷）的印刷、制卷厂家的选定和工序、质量的监控等工作要严格执行有关规定。

第四章 实施考试

第十八条 全区统一考试时间：高二年级第一学期末统考生物、历史、地理；高二年级第二学期末统考物理、化学、思想政治；高三年级第一学期末统考语文（蒙古语文甲）、数学、外语。具体考试时间报名时发布。

第十九条 学业水平考试要在标准化考场中进行，按照国家级考试要求组织，具体实施由各盟（市）教育招生考试机构按照《内蒙古自治区普通高中学业水平考试考点考场设置管理办法（暂行）》执行。

第二十条 学业水平考试按照属地管理的原则，盟（市）、旗（县、区）教育行政部门、教育招生考试机构和考点逐级负责，分工落实，领导、组织本考区、考点的考务工作，并按照《内蒙古自治区国家教育考试偶发事件处置办法（暂行）》和《内蒙古自治区国家教育考试突发事件应急处置预案（暂行）》处理考试期间发生的偶发、突发事件。

第二十一条 每个考点设主考、副主考各一人，主考由考点所在学校主要负责人担任。主考是考点的第一责任人，主考、副主考必须恪守《内蒙古自治区普通高中学业水平考试主考、副主考职责》，领导、组织监考员和其他工作人员严格按照《内蒙古自治区普通高中学业水平考试实施程序》施考。每个考场配备2名监考员，考场外设若干流动监考员。巡视员、监考员和其他工作人员必须恪守《全区各类教育考试自治区巡视人员工作纪律》、《内蒙古自治区普通高中学业水平考试监考员守则》和《内蒙古自治区普通高中学业水平

考试考点工作人员守则》。

第二十二条 考生按照《考生考试须知》参加考试，在考场内必须遵守《考生考试守则》，违纪舞弊者按照《国家教育考试违规处理办法》执行。

第二十三条 考试期间，自治区教育招生考试中心、盟（市）、旗（县、区）教育招生考试机构必须派巡视员到考区、考点，监督、检查考试实施情况。

第二十四条 考试期间，各级学业水平考试管理机构应安排昼夜值班，值班电话应在考前公布。遇有失密、泄密、考场大面积舞弊等重大问题，应立即报告自治区教育招生考试中心。

第二十五条 突发事件或异常情况的应对:

1.由于自然灾害等人力不可抗拒原因造成盟（市）、旗（县、区）未能及时实施学业水平考试，盟（市）、旗（县、区）教育招生考试机构应及时、逐级上报自治区教育招生考试中心，经批准后启用副题补考或另行组织考试。

2.因试卷丢失、被窃或其他原因造成试题失密、泄密，盟（市）、旗（县、区）教育招生考试机构必须立即报告或联系有关部门，采取有效措施控制扩散，并立即报告自治区教育招生考试中心。经查清失密、泄密范围后，则在失密、泄密范围内立即停止考试，并宣布此次考试结果无效。经批准后，重新组织考试。

3.启用副题或另行(重新)考试的组织管理、评卷等工作均依照本《实施办法》相应条款执行。自治区教育招生考试中心确定采取新的管理措施和考试成绩的等值处理。

4.在考试期间发现试卷有破损、印刷字迹不清楚等影响考生正常答题的情况时，经盟（市）教育招生考试机构请示自治区教育招生考试中心同意后，启用备用试卷进行考试。
