综合素质评价注意事项

1. 双击setup.exe文件，安装程序，安装过程中不要更改安装路径。默认安装路径为：d:\zhszpj2014\

2 . 程序打开密码：1

一、综合素质评价结果分为A（优）、B（良）、C（合格）、D（不合格）四个等级。A等的人数控制在初中毕业生总数的20—25%，B等控制在45—50%，其余35—25%为C或D等（D等一般不超过5%）。

二、注意事项：

1、各初中评价对象为参加中考的初三毕业生（凡中考报名的，考或不考均得评价）。评价表格用市中考综合素质评价程序打印。

2、各初中以班为单位对学生进行评价后，把评价结果分A、B、C、D四个等级填写在《2014年综合素质评价成绩册》“综合等级”一栏中。各等级人数所占百分比要符合评价要求。

3、各初中须把填写好的《2014年综合素质评价成绩册》复印后张贴在学校醒目位置对学生进行公示。

4、各初中须填写《2014年综合素质评价成绩统计表》（附后）。

5、各初中综合素质评价结果的电脑录入工作统一在教育股进行。

关于万荣中学文学名著阅读调查报告

摘要：名著阅读是学生学习和生活的重要组成部分。阅读名著在启发学生智力，道德素质培养和树立独立人格、提升思辨能力有着不可替代的作用。总之，名著阅读有助于培养学生的文化素养，促进学生的全面发展。本次调查主要从阅读兴趣、阅读时间和阅读环境三方面，对高中生的阅读现状展开研究。得出学生对名著阅读是有兴趣的，但迫于学校过分侧重高考成绩及学校图书馆方面建设的不足，学校教师在教学方法和教学目的的缺点，家庭方面也过分侧重学生的平时成绩，限制了学生阅读名著的兴趣的发展。并针对这些问题，从社会、学校、家庭三个方面提出一些实际可行的操作。改善学生的阅读环境，增强学生的阅读兴趣，帮助学生提高阅读水平和能力。从而促进学生的全面发展。

五、行程路线：沿孤峰街→宝鼎南路→后土东街→飞云南路→纪念碑→原 路返回。

六、活动要求：

1、安全教育。班主任在活动前教育学生注意交通安全，集体活动不准私自行动，防止走散。

2、养成教育。全体师生穿着校服，注意自身形象，坚持良好的卫生习惯，不随手乱丢纸屑，不乱吐痰。扫墓期间，严肃认真，不交头接耳，不嘻戏追逐。

3、以班为单位为学生每人做一朵小白花。各负责人认真督促各班做好准备工作。

4、身体不适的学生可以请假留在校内不参加扫墓活动，班主任做好安排。

5、扫墓仪式结束后，安排学生原路安全返校，切不可放任自流。

6、各班学生安全由班主任负责，代课教师协助。

7、扫墓活动结束后每班以缅怀先烈为主题选2篇优秀作文交德育处，择优推荐给校报，校报发表一篇加2分。

8、要求工作人员各负其责，认真组织、积极准备、创造特色、密切配合学校工作，确保这次扫墓活动顺利进行。

9、本次活动德育处统一考核，发现违纪现象每人扣3分。

七、活动流程：

4、学生代表发言（丁华鹏 辅导教师：李冰）。

5、校领导讲话。

6、主持人宣布活动结束，各班有序带回。
